Now do I just dig a hole and put it straight into the ground or do I need to take it out of the pot?

Under 99% of circumstances, you should:
1) Dig a hole at least as large as the pot
2) Add a layer of Miracle Grow or other fertilized soil to the bottom of the hole
3) Remove the plant from the pot
4) Place in hole
5) Fill in remaining space with fertilized dirt.
6) WATER. [When you plant a plant, it's like moving a person. The plants' roots are going to go into a small shock. Water eases the "pain."]
However, if you live in the Southwest or an arid area with highly alkaline soil, you're going to want to "plant" your plant in a larger pot and plant the entire pot in the soil. The larger pot will allow for root growth and your ability to control the pH levels of the soil. (i.e. planting Plumerias in Arizona)

Dig a hole, remove the potted plant out of the pot and then place it in the hole and cover any remaining space with soil.

If you don't take it out of the pot, the roots won't grow like they need to and it'll most likely die.
